 *Subscriptions/Memberships: Are non-transferable and non-refundable.  Members are elegible for services and discounts only if status is current and active.  No services will be performed if any balance still due.  Unused services can roll over up 2 months, account mut be active and current to use them.  Rollover services can not be used to cover cancellation agreement fee or past due balances. To obtain the discounts, all memberships are 6-12 months and may be paid annually, quarterly, or monthly. All clients are subject to the terms and conditions outlined in the Membership Agreement. Cancellations are via email info@ursulaversaceaesthetics.com with approval confirmation from the office.  Wax memberships, cancellation of agreement fee is $99.  Client agrees that this Authorization will remain in effect until they cancel it in writing, changes in my account information or termination of this authorization at least 15 days prior to the next billing date, or 15 days prior to the expiration date of the membership. Client acknowledges that the origination of Credit Card transactions to my account must comply with the provisions of U.S. law. Client certifies that they are an authorized user of this Credit Card and will not dispute these scheduled transactions; so long as the transactions correspond to the terms indicated in this authorization form.
