Subscriptions/Memberships: Are non-transferable and non-refundable. A valid card must be on file for monthly payment. Set up fee: $24.99

Members are elegible for services and discounts only if status is current and active.  Unused services will not roll over, account mut be active and current to use them.  Unused services can not be used to cover cancellation agreement fee or past due balances. To obtain the discounts, all memberships are 6 months and must be paid monthly.  VAlid card must be on file. All clients are subject to the terms and conditions. Clients’ membership will continue on an monthly basis until cancelled via email info@ursulaversaceaesthetics.com with approval confirmation from the office.  Facial memberships, cancellation of agreement fee is $199.00 and must be paid same day of cancelation and before next due date.  Wax memberships, cancellation of agreement fee is $99 and must be paid same day of cancelation and before next due date.  Client agrees that this Authorization will remain in effect until they cancel it in writing, and client agrees to notify Ursula Versace Aesthetics LLC in writing of any changes in my account information or termination of this authorization at least 10 days prior to the next billing date, or 10 days prior to the expiration date of the membership. Client acknowledges that the origination of Credit Card transactions to my account must comply with the provisions of U.S. law. Client certifies that they are an authorized user of this Credit Card and will not dispute these scheduled transactions; so long as the transactions correspond to the terms indicated in this authorization form.
